On the first day of school, your child must bring the following:
- Enrollment Packet
- Physical Form #3040 (within 30 days)
- Immunization Form #680 (within 30 days and must be current!)
- Emergency Medical
Authorization Form
- Authorization for Medication Form (as necessary for Physician
prescription only)
- (1) Current Pictures
- Change of Clothes (all labeled with child’s name)
- Naptime blanket/toy
- Infants/Non-Potty Trained Toddlers: Sufficient diapers/pull-ups/bottles/pacifiers/bibs/formula
(all labeled with child’s name)
We are not responsible for jewelry or personal toys brought
to the Center.
For the safety of all of our children, please do not use
beads in hair decorations since they are a choking hazard!
